Semester Internship

  • The department’s elective course "Off-Campus Internship" will be carried out during the second semester of the students’ senior year.
    For detailed information, please refer to the Off-Campus Internship Operation Process Table.
  • You may refer to institutions where previous senior students have completed internships; you may also search for institutions on your own.
    The prerequisite is: it must be related to the teaching content of this department, and you should consult and discuss it with your class advisor.
  • Documents to be completed by students
    • Course Authorization and Enrollment Consent Form:
      After completing this consent form, the course will be automatically added to the course registration system by the school, and students do not need to manually select the course.
    • Parental Consent Form:
      Students must discuss the summer internship information with their parents and submit the signed form.
    • Basic Information Form of the Internship Institution:
      Basic information of the institution.
    • Internship Training Plan:
      The training objectives, training items, and other details of this internship.
    • Internship Institution Evaluation Form:
      Divided into upper and lower sections; the upper section contains the institution’s basic information, and the lower section is completed by the instructor to evaluate the institution.
    • Off-Campus Internship Vacancy Form Provided by the Organization:
      Detailed description of the internship positions offered by the institution.
    • Off-Campus Internship Reflection Report:
      Students must submit it after the internship ends; please refer to the regulations for detailed requirements.
  • Documents to be completed by instructors
    • Internship Performance Evaluation Form by Supervising Instructor: The evaluation items include internship visit results and the completeness and richness of the reflection report, etc.
    • Instructor Internship Visit Record Form: During the summer internship, department instructors will conduct site visits and complete this record form.
  • Documents to be completed by organizations
    • Internship Institution Supervisor Evaluation Form: Evaluation items include internship learning attitude, professional ability, etc.
    • Internship Certificate: Certification of internship hours and related proof.
  • Semester Off-Campus Internship Contract : Two copies are required. After being stamped by both the institution and the school, each party keeps one copy.
  • Off-Campus Internship Cooperation Intention Form: Two copies are required. After being stamped by both the institution and the school, each party keeps one copy.
  • Internship Employer Satisfaction Questionnaire : At the end of the internship, the institution supervisor completes the form and the student submits it back to the school.
  • Student Internship Satisfaction Questionnaire : At the end of the internship, the student completes the form and submits it back to the school.
  • Among the graduating internship class, one student will serve as the person responsible for handling summer internship-related documents. If there are any questions, you may consult this student or inquire at the department office.
